If you go to the ER, you will eventually get treated and you will pay for it. A lot for it. Take your car instead of an ambulance and save 1K. They only treat people immediately if there is visible blood or loss of consciousness. Otherwise you may be in for a long wait in the waiting room until a bed opens up. The ER's main prerogative is to get a patient in a stable condition - ie, not in any obvious danger of dying, so their diagnostic tests may or may not be able to tell what wrong with him. They might give him pain meds on IV. After they see him in the ER (and get your billing info), they'll send your to a gastroenterologist or internist on call who will do some tests and treat you in that hospital wing.
